To sign up for the Suburban Child Summer Days program,
please complete this form, print out a copy for your records
and then submit. If you are sending a check, please enclose
a printed copy as well. If you have any questions,

Summer Days Deposit Policy
A Non-refundable $50 deposit must be paid for each session when registering. The remaining balance is due on a bi-weekly basis prior to your child's first week. It is not our policy to refund deposits or tuition.
If you
are paying by check, please print 2 copies of this completed form, one for your records AND one to send along with your check. Print and mail this application form along with your non-refundable deposit to:
Suburban Child, 10 Roxanna, Street, Framingham, MA 01702.
We will hold your child's spot for 5 business days from submission of this form.
If
you are paying by credit card, please print a copy
of this completed form for your records.
A parent packet will be mailed upon reciept of registration.
